Our providers serve South Dakota, North Dakota, and Iowa, in person and by telehealth. If you are leaving a hospital or crisis program, we can help with follow-up care so you are not navigating the next steps alone.

Yes. We provide follow-up and ongoing care after a crisis or hospital stay, including treating the underlying condition and supporting your safety plan.

No referral is needed to become a patient for ongoing care. You can reach out directly and we will help you get scheduled.

Yes. We offer secure video visits for ongoing care for patients located in South Dakota, North Dakota, and Iowa.
