How can I access my medical records or connect with my provider after an appointment?
MWI Health utilizes a secure patient portal for your convenience. Through the patient portal, you can securely access your medical records, view test results, manage prescriptions, and communicate directly with your healthcare team. Information on how to register for and access the patient portal will be provided during your initial visit, or you can find instructions on our website. For urgent matters between appointments, please call our office directly.
What should I bring to my first appointment at MWI Health?
Your insurance card and a valid photo ID.
A list of any current medications you are taking (including dosage and frequency).
Information about any past psychiatric medications you’ve tried and their effects.
Relevant medical records from previous mental health providers, if available.
A list of any questions or concerns you have about your mental health.
Any recent lab results or medical evaluations that might be relevant to your mental health. Arriving a few minutes early will allow you to complete any necessary intake forms.
